INDICATIONS & DOSAGE:

  • Prevention and Treatment of Blood clots in blood.
  • Unstable Angina
  • Prevention and treatment of Deep Vein Thrombosis (DVT) Prevention
  • Pulmonary Embolism (PE) Treatment
  • Thromboprophylaxis in High-Risk Surgical Patients
  • Non-ST Elevation Myocardial Infarction (NSTEMI)
  • ST-Elevation Myocardial Infarction (STEMI)
  • Acute Coronary Syndrome (ACS)

PACK SIZE: 1X2’s PFSs

DOSAGE & ADMINISTRATION: Administer via subcutaneous injection, alternating between left and right anterolateral and left and right posterolateral abdominal wall.

Prophylaxis of Deep Vein Thrombosis (DVT):
  • General Surgery: 40 mg subcutaneously once daily, starting 2 hours before surgery.
  • Medical Patients: 40 mg subcutaneously once daily.

Treatment of DVT with or without Pulmonary Embolism (PE):
  • 1 mg/kg subcutaneously every 12 hours, or 1.5 mg/kg subcutaneously once daily.
Unstable Angina and Non-Q-Wave Myocardial Infarction:
  • 1 mg/kg subcutaneously every 12 hours, with aspirin.

Acute ST-Segment Elevation Myocardial Infarction (STEMI):
  • Initial dose of 30 mg IV bolus plus 1 mg/kg subcutaneously, followed by 1 mg/kg subcutaneously every 12 hours.
  • For patients ≥75 years, no bolus; 0.75 mg/kg subcutaneously every 12 hours.

Percutaneous Coronary Intervention (PCI):
  • If the last subcutaneous dose was given less than 8 hours before balloon inflation, no additional dosing needed.
  • If the last subcutaneous dose was given 8 to 12 hours before balloon inflation, an additional 0.3 mg/kg IV bolus should be administered.

Renal Impairment:
  • For patients with severe renal impairment (creatinine clearance <30 mL/min), the dosage should be adjusted.
  • Prophylaxis of DVT: 30 mg subcutaneously once daily.
  • Treatment of DVT, PE, unstable angina, and non-Q-wave MI: 1 mg/kg subcutaneously once daily.
